Call our office or send an email letting us know you’re seeking services as a New Patient. Be sure to specify if you are looking for telehealth or in-person services. A member of our staff will help create your patient profile and schedule your initial appointment. Please have your health insurance plan name and member ID number available when calling to schedule.
*Failure to submit Intake Forms 24hrs prior to scheduled appointments may be subjected to cancellation.
